Testicular Descent: When to Interfere?

Summary

Spontaneous descent of testes after birth can occur in up to 70 % of cases, yet the factors contributing to it are still controversial. This study aims to evaluate factors contributing to spontaneous descent of palpable undescended testes. Eighty-four newborns with 126 palpable undescended testes (42 unilateral and 42 bilateral) were followed up for a period of one year to study the occurrence and time of testicular descent and its relation to gestational age, birth weight, uni- or bilaterality and levels of FSH, LH and testosterone. A total of 58 testes (46 %) descended between 3 and 6 months. Spontaneous descent occurred in 10 premature patients (14 testes 63 %) compared to 44 testes of full-term patients (43 %). Descent occurred in 14 unilateral undescended testes (33 %) compared to 44 (52 %) in bilateral cases. In patients with spontaneous testicular descent there was postnatal peak of LH and testosterone at 2 to 3 months of age which returned to basal level at 6 months of age. In patients with permanent undescended testes the peak of LH and testosterone was very low and almost absent in some of them; no significant difference was found between the mean values of FSH in both groups. No spontaneous testicular descent occurred after the 4th month in the full-term group, whereas in the pre-term group spontaneous descent occurred up to 6 months of age. This study concluded that spontaneous descent of palpable undescended testes is closely related to the presence of LH and testosterone surge. Therapy of undescended testes should start at 4 months of age in a full-term baby and at 6 months of age in a pre-term baby.

La migration spontanée des testicules après la naissance peut survenir dans 70 % des cas. Les facteurs contribuant à cette migration ne sont pas tout à fait connus. Le but de cette étude est d'évaluer les facteurs contribuant à la migration spontanée des testicules. 84 nouveau-nés avec 126 testicules non palpables (42 unilatéraux et 42 bilatéraux) ont été surveillés pendant une année pour étudier la survenue et la durée de la migration et ses relations avec l'âge gestationnel, le poids de naissance, le caractère uni ou bilatéral, et les taux de FSH, LH et testostérone. 58 patients (46 %) ont eu une migration spontanée entre 3 et 6 mois. La migration spontanée est survenue chez 10 enfants prématurés (14 testicules, 63 %), alors qu'elle était observée chez 44 enfants (43 %) nés à terme. La migration a été observée dans 14 (33 %) ectopies testiculaires unilatérales et dans 44 (52 %) forme bilatérale. Chez les enfants avec migration spontanée, il y avait une élévation des taux de LH et de testostérone du 2ème au 3ème mois, avec retour au niveau de base à l'âge de 6 mois. Chez les enfants avec une ectopie persistante, le taux de LH et de testostérone était très bas. Aucune différence n'est observée pour les taux de FSH entre les deux groupes. Il n y'a pas de migration testiculaire spontanée après l'âge de 4 mois chez les enfants nés à terme, alors qu'elle peut être observée jusqu'à l'âge de 6 mois chez les enfants prématurés. En conclusion, la migration testiculaire spontanée est liée à la présence d'une élévation des taux de LH et de testostérone. Le traitement des testicules non descendues doit être commencé à l'âge de 4 mois chez les enfants à terme et à l'âge de 6 mois chez les enfants prématurés.

Resumen

El descenso espontáneo de los testículos tras el nacimiento puede ocurrir en el 70 % de los casos pero los factores que contribuyen a él son todavía controvertidos. Este estudio pretende evaluar los factores que contribuyen al descenso espontáneo de testículos no descendidos palpables. 84 recién nacidos con 126 testículos no descendidos palpables (42 unilaterales y 42 bilaterales) fueron seguidos por un periodo de un año para estudiar la aparición y el momento del descenso testicular y su relación con la edad gestacional, con el peso natal, con la uni o bilateralidad y con los niveles de FSH, LH y testosterona. Un total de 58 testículos (46 %) descendieron entre 3 y 6 meses. El descenso espontáneo ocurrió en 10 pacientes prematuros (14 testículos, 63 %) en comparación con 44 testículos de niños a término (43 %). El descenso ocurrió en 14 testículos maldescendidos unilaterales (33 %) en comparación con 44 (52 %) de los casos bilaterales. En pacientes con descenso testicular espontáneo había un pico postnatal de LH y testosterona a los 2 o 3 meses de edad que volvió a la normalidad a los 6 meses. En pacientes con testículos permanentemente maldescendidos el pico de LH y testosterona fue muy bajo y casi ausente en algunos de ellos sin que hubiera diferencias significativas entre los valores medios de FSH en ambos grupos. No hubo descenso testicular espontáneo después del 4º mes de vida en el grupo de pacientes a término mientras que en el grupo de pretérminos hubo descensos hasta los 6 meses de edad. Este estudio concluye que el descenso espontáneo de testículos no descendidos palpables está muy relacionado con la presencia de elevaciones de los niveles de LH y testosterona. La terapéutica de los testículos descendidos debe comenzar a los 4 meses de edad en los pacientes a término y después de los 6 en los pretérminos.

Zusammenfassung

Ein spontaner Descensus testiculorum nach der Geburt kann in 70 % der Fälle vorkommen. Die Faktoren, die zum spontanen Deszensus beitragen, sind immer noch umstritten. Diese Studie hat zum Ziel, die Faktoren herauszufinden, die zum spontanen Deszensus von fühlbaren, nicht deszendierten Hoden führen. 84 Neugeborene mit 126 fühlbaren, nicht abgestiegenen Hoden (42 unilateral und 42 bilateral) wurden während eines Jahres beobachtet, um das Eintreten und den Zeitpunkt des Hodenabstiegs und den Zusammenhang mit dem Fetal-Alter, dem Geburtsgewicht, der Unilateralität oder Bilateralität und dem Spiegel von FSH, LH und Testosteron zu untersuchen. 58 Hoden (46 %) stiegen nach 3 bis 6 Monaten ab. Spontaner Deszensus erfolgte bei 10 Frühgeburten (14 Hoden; 63 %) verglichen mit 44 Hoden bei normalgeborenen Patienten (43 %). Ein Deszensus trat ein bei 14 unilateralen nicht abgestiegenen (deszendierten) Hoden (33 %) verglichen mit 44 (52 %) bei bilateralen Fällen. Bei Patienten mit spontanem Deszensus lag der Höchstwert von LH und Testosteron nach der Geburt vor, nämlich im Alter von 2 bis 3 Monaten. Im Alter von 6 Monaten fiel der Wert auf den Ausgangswert zurück. Bei Patienten mit ständig nicht deszendierten Hoden waren die Maximalwerte von LH und Testosteron sehr niedrig, in einigen Fällen sogar fast nicht vorhanden. Keinen nennenswerten Unterschied gab es in beiden Gruppen zwischen den Durchschnittswerten des FSH. Kein Descensus testiculorum erfolgte nach dem 4. Lebensmonat in der Gruppe der ausgetragenen Fälle (full-term group), während in der Gruppe der Frühgeburten bis zum 6. Lebensmonat ein spontaner Deszensus erfolgte. Diese Studie zeigt, dass der spontane Abstieg von fühlbaren, nicht deszendierten Hoden vom Vorhandensein des LH- und Testosteron-Höchstwertes abhängt. Eine Therapie von nicht deszendierten Hoden sollte bei ausgetragenen Babies (reif geborenen) im 4. Monat und bei Frühgeburten im 6. Monat beginnen.
