Transient Pseudohypoaldosteronism Secondary to Posterior Urethral Valves - A Case Report and Review of the Literature

Summary

In transient pseudohypoaldosteronism (TPHA), renal tubular resistance to aldosterone is thought to be secondary to renal disease. We report a case of TPHA caused by posterior urethral valves associated with urinary tract infection and review 62 cases previously reported. The infant presented with unspecific signs of vomiting and dehydration, so that pyloric stenosis was first suspected. Laboratory data and retroperitoneal sonography led to the diagnosis TPHA. This case illustrates that urine culture and renal ultrasonography should be performed in any infant with electrolyte disturbances to exclude infection or obstructive uropathy.

Résumé

Dans le pseudohypoaldosteronisme transitoire (TPHA) la résistance tubulaire à l'aldostérone est censée être secondaire à une altération rénale. Nous reportons un cas de TPHA causé par des valves de l'urètre postérieur associé à une infection urinaire et revoyons les 62 cas précédemment rapportés. L'enfant présentait des signes non spécifiques: vomissements et déshydratation, qui ont fait suspecter une sténose du pylore. Les examens biologiques et l'échographie rétro péritonéale ont conduit au diagnostic de TPHA. Ce cas illustre qu'un examen cytobactériologique des urines et une échographie rénale doivent être réalisés chez chaque enfant présentant des troubles électrolytiques pour exclure une infection ou une uropathie obstructive.

Resumen

La resistencia tubular renal a la aldosterona en el pseudohipoaldoesteronísmo transitorio (TPHA) se considera secundaria a la enfermedad renal. Presentamos un caso de TPHA causado por válvulas de uretra posterior asociadas a infección del tracto urinario y revisamos 62 casos previamente publicados. El niño tenía signos inespecíficos de vómitos y de deshidratación que hicieron sospechar primeramente una estenosis pilórica. Los datos de laboratorio y la ultrasonografía retroperitoneal permitieron el diagnóstico de TPHA. Este caso ilustra que el cultivo de orina y la ecografía deben ser realizadas en cualquier niño con trastornos electrolíticos para excluir la infección o la uropatía obstructiva.

Zusammenfassung

Die Ursache des transienten Pseudohypoaldosteronismus (TPHA) ist vermutlich eine erworbene Resistenz des Nierentubulus für Aldosteron. Wir stellen einen Fall von TPHA, verursacht durch einen Harnwegsinfekt bei Harnröhrenklappen, vor und geben einen Überblick über 62 bereits in der Literatur beschriebene Fälle. Der Säugling wurde mit Erbrechen, Gedeihstörung und Dehydration in die Klinik gebracht, so dass zunächst an eine hypertrophe Pylorusstenose gedacht wurde. Die Urinkultur und die Sonographie des Harntraktes führten zur Diagnose TPHA. Dieser Fall macht deutlich, dass bei jedem Kind mit Elektrolytverschiebungen eine Urinuntersuchung und ein Ultraschall der Nieren und der Harnblase unbedingt notwendig sind, um Harnwegsinfekte und obstruktive Uropathien auszuschließen.
