Pitfalls in Diagnosing Carpal Tunnel Syndrome

Abstract

The aim of this paper is to point out the differential diagnosis between a thumb hypoplasia type I/II and a carpal tunnel syndrome. Both syndromes have symptoms in common. Two children and an adult with hypoplastic thumbs were sent to our department with the diagnosis carpal tunnel syndrome. In both children, the diagnosis was changed after an exact history and examination, and in one child an opponensplasty was done. The adult patient had already been operated unsuccessfully for a carpal tunnel syndrome elsewhere and again at our department. The exact diagnosis was only made after the second operation, when an extended questioning was done. This shows how important a careful history taking, an exact clinical examination, X-ray examinations of both hands, knowledge of type I/II hypoplasia, and finally electroneurography and sonography are important for a correct diagnosis.

Résumé

Le but de ce papier est de préciser les différents diagnostics entre une hypoplasie du pouce de type I/II et un syndrome du canal carpien. Ces deux syndromes peuvent avoir des symptômes en commun. Deux enfants et un adulte avec une hypoplasie des pouces sont envoyés dans notre département avec le diagnostic de syndrome du canal carpien. Chez les deux enfants, le diagnostic était modifié après examen. Chez un enfant, une plastie d'opposition était faite. Le patient adulte avait déjà été opéré sans succès du syndrome du canal carpien ailleurs, et de nouveau dans notre département. Le diagnostic exact était fait seulement après la seconde opération quand un interrogatoire précis était fait. Ceci montre combien il est important d'avoir une histoire précise, un examen clinique exact, des radios des deux mains et une connaissance de l'hypoplasie de type I/II et éventuellement une électroneurographie et une échographie sont nécessaires pour un diagnostic correct.

Resumen

El objetivo de este trabajo es señalar el diagnóstico diferencial entre la hipoplasia del pulgar tipo I y II y el síndrome del tunel carpiano. Ambos síndromes tienen síntomas comunes. Tratamos dos niños y un adulto con pulgares hipoplásicos con el diagnóstico de síndrome del tunel carpiano. En los dos, el diagnóstico fue cambiado tras una historia y un examen clínico cuidadoso haciéndose una plastia del oponente en uno de ellos. El paciente adulto ya había sido operado sin éxito por un síndrome del tunel carpiano en otro lugar y también en nuestro propio departamento. El diagnóstico exacto sólamente fue posible después de esta segunda operación cuando se hizo un interrogatorio cuidadoso. Estos pacientes muestran la importancia de una historia detallada y de un examen igualmente minucioso de las radiografías de ambas manos, del conocimiento de la hipoplasia tipo I/II y eventualmente de la electroneurografía y de la sonografía para el diagnóstico correcto.

Zusammenfassung

Das Ziel dieser Arbeit ist das Aufzeigen der Wichtigkeit der Differenzialdiagnose zwischen einer Daumenhypoplasie Grad I/II und einem Karpaltunnelsyndrom. Beide Syndrome haben gemeinsame Symptome. Zwei Kinder und eine Erwachsene mit einer Daumenhypoplasie wurden uns mit der Diagnose Karpaltunnelsyndrom zugewiesen. Bei beiden Kindern wurde die Diagnose nach einer ausführlichen Anamnese und klinischen Untersuchung richtiggestellt, bei einem Kind wurde eine Opponensplastik durchgeführt. Die erwachsene Patientin war bereits auswärts und ein zweites Mal bei uns wegen des Verdachts auf Karpaltunnelsyndrom erfolglos operiert worden, erst nach dem zweiten Eingriff wurde aufgrund der nachträglichen gezielten Anamnese die Diagnose richtig gestellt. Eine genaue Anamnese, exakte klinische Untersuchung, ein Handröntgen im direkten Seitenvergleich, das Wissen um eine Daumenhypoplasie und eventuell zusätzlich eine elektroneurographische und sonographische Untersuchung sind wichtig zur Erstellung der richtigen Diagnose.
