Skin Level Division of Percutaneous Endoscopic Gastrostomy Without Endoscopy Retrieval: A Hazardous Procedure

Abstract

Percutaneous endoscopic gastrostomy (PEG) has become the method of choice for long-term enteral access in the pediatric population. Since its introduction, several common complications have been described ([3]). Less well known is the danger of removing or replacing a PEG by cutting the device at skin level without endoscopic assistance to ensure the complete removal of all parts. The aim of the present work is to describe a patient in whom gastrostomy parts were retained after PEG removal, causing bowel obstruction and perforation.

Résumé

La gastrostomie endoscopique per-cutanée (PEG) est devenue la méthode de choix pour l'accès entéral à long terme chez l'enfant. Depuis son introduction, plusieurs complications ont été décrites. Moins connu est le danger d'enlever et de replacer une PEG en coupant la prothèse au niveau de la peau sans l'aide d'une endoscopie pour s'assurer de l'absence de parties laissées en place. Le but de ce travail est de décrire un patient chez lequel le matériel de gastrostomie était resté après ablation du PEG causant une obstruction intestinale et une perforation.

Resumen

La gastrostomía endoscópica percutánea (PEG) se ha convertido en el método de elección para el acceso enteral a largo plazo en niños. Desde su introducción se han descrito varias complicaciones. La menos conocida es el peligro de extraer o sustituir la sonda cortándola a ras de la piel sin asistencia endoscópica para asegurarnos de que no queden partes retenidas. El objeto del presente trabajo es describir un paciente en quien piezas de esta sonda de gastrostomía fueron retenidas tras la extracción de la misma causando obstrucción intestinal y perforación.

Zusammenfassung

Die PEG ist zur Behandlungsmethode der Wahl bei Langzeit-enteraler Ernährung im Kindesalter geworden. Seit ihrer Einführung wurden jedoch zahlreiche Komplikationen beschrieben ([3]). Weniger bekannt sind Gefahren, die bei der Entfernung oder beim Auswechseln der PEG-Sonde entstehen können, z. B. dadurch, dass die Sonde in Hautniveau durchgeschnitten wird. Wenn nicht alle Teile der PEG endoskopisch entfernt werden, kann dies zur intestinalen Obstruktion im Sinne eines Ileus führen. Der Fall eines 4,5 Jahre alten Knaben wird vorgestellt.
