Indications and Results of Surgery in Patients with Crohn's Disease with Onset under 10 Years of Age: A Series of 18 Patients

Abstract

In order to examine the presentation and course of Crohn's disease (CD) with an onset of CD before the age of 10 and to assess indications and effects of surgery, a retrospective study was performed in 18 children.

One hundred and twenty-two children with CD have been followed over the last 22 years in our institution. Thirty of them had the first onset of disease before 10 years of age. Eighteen out of these 30 underwent intestinal resection (IR). The mean age at the onset of CD was 6.4 years and the average duration of follow-up was 11.1 years. Most common symptoms were abdominal pain (72 %), diarrhoea (72 %), and growth retardation (50 %). Colon and ileon were the most frequently involved areas. Most of the children (94.5 %) received extensive medical and or nutritional treatment before surgery. We performed 26 IRs and 14 other operations in 18 patients. Indications for IR were chronic intestinal obstruction (13 cases) and chronic intestinal disability (13 cases). Improvements in the general clinical state were obtained in 17 cases out of 18. IR is beneficial for chronically ill children with ineffective medical therapy. A disease-free interval after surgery may provide an important time for improved growth and pubertal development and a return to nearly normal life for a long time.

Dans le but d'étudier le mode de présentation et l'évolution de la maladie de Crohn (CD) avec un début avant l'âge de 10 ans et d'apprécier les indications et les effets de la chirurgie, une étude rétrospective a été réalisée chez 18 enfants. Cent vingt-deux enfants avec CD ont été traités pendant une Période de 22 ans dans notre institution. Trente ont eu le premier signe de la maladie avant l'âge de 10 ans. Dix-huit parmi les trente ont eu une résection intestinale (IR). L'âge moyen AU début de CD était de 6.4 ans et la durée moyenne du suivi était de 11.1 ans. Les symptômes les plus communs étaient de douleurs abdominales (72 %), des diarrhées (72 %) et des retards de croissance (50 %). Le côlon et l'iléon étaient le plus fréquemment concernés. La plupart de ces enfants (94,5 %) recevaient un traitement médical intensif et/ou un traitement nutritionnel avant la chirurgie. Nous avons réalisé 26 résections intestinales et 14 autres opérations chez ces 18 patients. L'indication pour IR était l'obstruction intestinale chronique (13 cas) ou un non fonctionnement intestinal chronique (13 cas). Une amélioration de l'état clinique était obtenue chez 17 des 10 cas. IR est bénéfique chez les enfants pour lesquels le traitement médical est inefficace. Un intervalle libre après la chirurgie peut entraîner un moment important pour améliorer la croissance, le développement de la puberté et pour retourner à une vie presque normale pour un long moment.

Hemos realizado un estudio retrospectivo sobre la presentación y el curso clínico de la enfermedad de Crohn (CD) de comienzo anterior a los 10 años para evaluar las indicaciones y los efectos de la cirugía.

En los últimos 22 años hemos visto 122 niños con CD en nuestra institución. 30 de ellos tuvieron un comienzo clínico antes de los 10 años. 18/30 requirieron resección intestinal. La edad media al comienzo fue de 6.4 años y la duración media del seguimiento de 11.1 años. Los síntomas más comunes fueron dolor abdominal (72 %), diarrea (72 %) y retraso del crecimiento (50 %). Las áreas más frecuentemente afectadas fueron el colon y el ileon. La mayoría de los niños (94.5 %) fueron tratados vigorosamente con medidas médicas y nutritivas antes de la cirugía. Realizamos 26 resecciones intestinales y 14 operaciones de otra índole en 18 pacientes. Estas operaciones se hicieron prácticamente todas con carácter programado. Las indicaciones de la resección intestinal fueron obstrucción crónica (13 casos) o insuficiencia intestinal crónica (13 casos). Se obtuvo mejoría del estado clínico en 17/18 pacientes.

Los efectos de la resección intestinal son favorables en los niños crónicamente enfermos con terapia médica ineficaz. Un intervalo libre tras la cirugía puede proporcionar un momento óptimo para mejorar el crecimiento y el desarrollo puberal y para una vuelta a la normalidad durante un periodo largo de tiempo.

Zusammenfassung

Um das Erscheinungsbild und den Verlauf des Morbus Crohn (MC) bei einer Krankheitsmanifestation vor dem 10. Lebensjahr zu untersuchen und Indikationen und Wirkungen eines operativen Eingriffes abzuschätzen, wurde eine retrospektive Studie an 18 Kindern durchgeführt.

122 Kinder mit MC sind in den letzten 22 Jahren in unserer Einrichtung behandelt worden. Bei 30 von ihnen manifestierte sich die Krankheit vor dem 10. Lebensjahr. Bei 18 dieser 30 wurde eine Darmresektion durchgeführt (DR). Das Durchschnittsalter bei Ausbruch des MC war 6,4 Jahre, die Durchschnittsdauer der Nachbeobachtung betrug 11,1 Jahre. Die häufigsten Symptome waren Bauchschmerzen (72 %), Durchfall (72 %) und Wachstumsverzögerung (50 %). Kolon und Ileum waren die am häufigsten betroffenen Gebiete. Die meisten Kinder (94,5 %) erhielten präoperativ medikamentöse und/oder diätetische Behandlung. Wir führten 26 Darmresektionen und 14 andere Operationen durch. Indikationen für die Darmresektion waren chronische Darmobstruktion (12 Fälle) und chronische intestinale Dysfunktion (12 Fälle). In 17 von 18 Fällen wurde eine Verbesserung erreicht.

Die Wirkungen einer Darmresektion sind vorteilhaft bei chronisch kranken Kindern mit wirkungsloser medikamentöser Therapie. Ein krankheitsfreies Intervall nach der Operation kann einen wichtigen Zeitraum zur Verbesserung des Wachstums und der pubertären Entwicklung liefern und für lange Zeit eine Rückkehr zu einem fast normalen Leben ermöglichen.
