Stenting of the Biliary Tract in Children

Abstract

We report on the technique and results of percutaneous transhepatic biliary drainage (PTBD) in children with obstructive jaundice. Three patients aged 8 - 15 years were treated, two of them for a benign and one for a malignant stricture. Endoscopic treatment was not possible and all the PTBD procedures were done under general anaesthesia. One of the children was treated with external-internal drainage, and the two others by insertion of a plastic endoprosthesis. There were no immediate complications. The PTBD had a good palliative effect in two cases, and in one case surgical treatment was necessary. We conclude that PTBD is a safe modality and that it can be used in children for the relief of obstructive jaundice.

Résumé

Nous rapportons la technique et les résultats du drainage per-cutané trans-hépatique (PTBD) chez l'enfant qui présente une choléstase. Trois patients âgés de 8 à 15 ans ont été traités, deux pour une lésion bénigne et un pour une sténose maligne. Le traitement endoscopique n'était pas possible et tous les PTBD ont été réalisés sous anesthésie générale. Un de ces enfants était traité avec un drainage externe et les deux autres par insertion d'une endo-prothèse en plastique. Il n'y avait aucune complication immédiate. Le PTBD a eu un bon effet palliatif dans deux cas et dans un cas un traitement chirurgical a été nécessaire. Nous concluons que le PTBD est un procédé sûr et qu'il peut être utilisé chez l'enfant pour le traitement des choléstases.

Resumen

Presentamos la técnica y los resultados del drenaje biliar transhepático percutáneo (PTBD) en niños con ictericia obstructiva.

Tratamos tres pacientes con edades entre 8 y 15 años dos de los cuales tenían estenosis benignas y uno maligna. El tratamiento endoscópico no fue posible y todos los procedimientos de PTBD se realizaron bajo anestesia general. Uno de los pacientes fue tratado con drenaje interno y externo y los otros dos con inserción de una endoprótesis plástica. No hubo complicaciones inmediatas. El PTBD tuvo un efecto paliativo bueno en 2 casos y en otro fue necesario el tratamiento quirúrgico.

Concluímos que el PTBD es una modalidad segura del tratamiento y que puede ser usado en niños para el alivio de la ictericia obstructiva.

Zusammenfassung

Es wird über die Technik und das Ergebnis einer perkutanen, transhepatischen Gallenwegsdrainage bei drei Kindern im Alter von 8 - 15 Jahren berichtet, die unter einer Gallenwegsobstruktion litten. Bei zwei Kindern lag eine benigne, bei einem eine maligne Erkrankung vor. Während im Erwachsenenalter endoskopische Stenteinlagen bei malignen Erkrankungen die Regel sind, sind sie bei Kindern die Ausnahme. Stenteinlagen sind jedoch bei Kindern schwierig. Auch bei den vorliegenden Patienten war eine endoskopische Anlage nicht möglich, SO dass eine perkutane Gallenwegsdrainage unter Allgemeinanästhesie durchgeführt wurde. Ein Kind wurde mit einer Extern-intern-Drainage, zwei weitere mit einer Plastikendoprothese versorgt. Frühkomplikationen konnten nicht beobachtet werden. Die perkutane, transhepatische Drainage ist eine gute palliative Operation, die bei zwei Kindern endoskopisch, bei einem offen chirurgisch durchgeführt werden konnte.
