Preoperative Catheterization of H-Type Tracheoesophageal Fistula to Facilitate its Localization and Surgical Correction

Abstract

Purpose: The aim of this study was to demonstrate that preoperative catheterization of H-type tracheoesophageal fistula facilitates its identification and surgical correction. Methods: This is a case series of seven patients with H-type tracheoesophageal fistula. Diagnosis was established in three patients and suspected in two more by means of an esophagogram. Confirmation of fistula was performed by endoscopy in all patients. On the day of surgery, either a rigid bronchoscope or a nasolaryngoscope was introduced into the trachea to localize and catheterize the fistula. Because of the location of the fistula, the surgical correction was performed through the neck in five patients and through the thorax in two patients. The fistulas were easily identified surgically and the corrections were successfully performed in all cases. An extensive dissection was not required in any case. Refistulization, identified by bronchoscopic examination one week after surgery, occurred in one patient. Surgical repair was performed again using same procedure described above. All patients are currently asymptomatic and without any evidence of refistulization. Conclusions: Preoperative catheterization of H-type tracheoesophageal fistula is useful to facilitate its preoperative identification, to plan the surgical approach, and to decrease operating times and the extent of surgery.

But: Démontrer que le cathétérisme pré-opératoire des fistules trachéo-oesophagiennes en «H» facilite son identification et la correction chirurgicale. Méthode: C'est une série de 7 patients avec une fistule trachéo-oesophagienne en «H». Le diagnostic était établi chez 3 patients, suspecté chez 2 par un oesophagogramme. La confirmation de la fistule était réalisé par endoscopie chez tous les patients. Le jour de l'opération soit un bronchoscope rigide ou un naso-laryngoscope était introduit dans la trachée pour localiser et cathétériser la fistule. A cause de la localisation de la fistule, la correction chirurgicale était réalisée par une voie cervicale chez cinq patients et par une voie thoracique chez deux. Les fistules étaient faciles à identifier chirurgicalement et la correction était réalisée avec succès dans tous les cas. Une dissection extensive n'est pas nécessaire dans ce cas, une refistulisation identifiée par un examen bronchoscopique une semaine après la chirurgie était observée chez un patient. L'exploration chirurgicale était réalisée à nouveau avec la même technique que celle utilisée précédemment. Tous les patients sont asymptomatiques et sans évidence de refistulisation. Conclusion: Le cathétérisme pré-opératoire des fistules trachéo-oesophagiennes en «H» est utile pour faciliter son identification pré-opératoire pour faciliter l'approche chirurgicale et diminuer le temps chirurgical et la dissection.

Resumen

Objetivo: Demostrar que la cateterización preoperatoria de la fístula traqueoesofágica en H facilita su identificación y corrección quirúrgica. Métodos: Presentamos 7 pacientes con fístula traqueoesofágica en H. El diagnóstico se estableció por esofagograma en 3 pacientes y se sospechó en 2. La confirmación de la presencia de la fístula se realizó por endoscopia en todos los pacientes. En el día de la operación utilizando o un broncoscopio rígido o un nasolaringoscopio introducido en la tráquea se localizó y cateterizó la fístula. En función de su localización se realizó la operación a través del cuello en 5 pacientes o del tórax en 2. Las fístulas fueron fácilmente identificadas quirúrgicamente y la corrección tuvo éxito en todos los casos. No fue necesaria una disección extensa en ningún caso. Hubo una refistulización identificada por broncoscopia una semana después de la cirugía. El tratamiento quirúrgico se realizó otra vez mediante el mismo procedimiento descrito arriba. Todos los pacientes están actualmente asintomáticos y sin evidencia de refistulización. Conclusiones: El cateterismo preoperatorio de la fístula traqueoesofágica en H es útil para facilitar su identificación preoperatoria, para planear el abordaje quirúrgico y para disminuir el tiempo quirúrgico y la extensión de la disección.

Zusammenfassung

Zielsetzung: Ziel der Untersuchung war es zu zeigen, dass eine präoperative Katheterisierung einer tracheo-ösophagealen H-Fistel die Identifizierung der Fistel während des chirurgischen Eingriffs erleichtert. Methode: Sieben Patienten mit tracheo-ösophagealen H-Fisteln wurden untersucht. Die Diagnose wurde bei drei Kindern primär radiologisch durch ein Ösophagogramm gestellt, bei 2 weiteren radiologisch nur vermutet. Endoskopisch konnte jedoch die Diagnose bei allen Kindern gestellt werden. Am Tage der Operation wurde entweder mit einem starren Bronchoskop oder einem flexiblen Nasolaryngoskop die Fistelöffnung eingestellt und die Fistel katheterisiert. Aufgrund dieser Lokalisation der Fistel wurde anschließend bei 5 Patienten vom Hals aus, bei 2 weiteren transthorakal vorgegangen. Die Fisteln konnten leicht identifiziert und der Eingriff in allen Fällen leicht durchgeführt werden. In keinem Fall war eine ausgedehnte Revision erforderlich. Bei einem Kind kam es eine Woche nach dem Eingriff zu einer bronchoskopisch gesicherten Rekanalisation der Fistel. Die Fistel wurde auf die gleiche Weise wie oben angegeben erneut verschlossen. Alle Patienten sind zurzeit asymptomatisch und ohne Hinweis auf ein Fistelrezidiv. Schlussfolgerungen: Die präoperative Katheterisierung einer tracheo-ösophagealen H-Fistel ist nützlich für den präoperativen Nachweis der Fistel, die Wahl des chirurgischen Zugangs und um Operationszeit und ‐umfang zu sparen.
