Management of Childhood and Adolescent Thyroid Carcinoma: Long-Term Follow-Up and Clinical Characteristics

Abstract

Aim: This analysis was performed to evaluate clinical factors such as local tumor control, control of distant metastasis, survival, and complications in children and adolescents with thyroid carcinoma. Materials and Methods: From 1960 to 2003, 21 children and adolescents (16 girls and 5 boys, aged 4 to 15 years) were treated for thyroid carcinoma in our departments. Clinical data, operative findings, pathological findings, operative procedures, intra- and postoperative complications and follow-up were evaluated. Follow-up of patients ranged from 3 to 42 years (median, 20 years). Results: Papillary carcinoma was found in 20 patients (95 %) and follicular carcinoma in one patient. Cervical lymph node metastases were found in 12 out of 21 patients (57.1 %) when thyroid carcinoma was diagnosed, and 5 patients (23.8 %) had pulmonary metastasis. In patients with pulmonary metastasis, total thyroidectomy and bilateral radical lymph node resection was performed in 3 patients, lobectomy of the thyroid and radical lymph node resection in one patient, and partial thyroidectomy in one patient. All three patients who underwent total thyroidectomy are still alive. Internal irradiation by 131I and thyroid hormone replacement therapy was administered to 2 of 3 patients with pulmonary metastasis. Conclusion: Despite metastasis in the lymph nodes and even the lungs, the prognosis for patients with papillary carcinoma is good. Total thyroidectomy and positive 131I therapy are recommended for childhood and adolescent thyroid carcinoma with pulmonary metastasis.

Cette analyse a été réalisée pour évaluer les éléments cliniques tels que le contrôle tumoral local, contrôle des métastases, survie et complications chez les enfants et les adolescents avec un carcinome de la thyroïde. Matériel et Méthodes: De 1960 à 2003, 21 enfants et adolescents (16 filles et 5 garçons âgés de 4 à 15 ans) ont été traités pour un carcinome thyroïdien dans notre département. La clinique, les découvertes opératoires, les découvertes pathologiques, les techniques chirurgicales, les complications intra et post-opératoires et le suivi ont été évalués. Le suivi des patients s'étalait de 3 à 42 ans (médiane 20 ans). Résultats: Le carcinome papillaire était retrouvé chez 20 patients (95 %) et un carcinome folliculaire chez un patient. L'incidence des métastases ganglionnaires cervicales était de 12 parmi les 21 (57,1 %) quand un diagnostic de carcinome de la thyroïde était fait et 5 (23,8 %) avaient des métastases pulmonaires. Chez les patients avec métastases pulmonaires, une thyroïdectomie totale et un curage ganglionnaire cervical bilatéral ont été réalisés chez 3 patients, une lobectomie de la thyroïde et un curage ganglionnaire radical chez un patient et une thyroïdectomie partielle chez un patient. Trois patients qui ont eu une thyroïdectomie totale sont toujours en vie. Irradiation par iode 131 et thérapie de substitution par hormones thyroïdiennes étaient administrées chez deux des trois patients avec des métastases pulmonaires. Conclusion: Malgré les métastases dans les ganglions et les métastases pulmonaires, le pronostic des patients avec un carcinome papillaire est bon. La thyroïdectomie totale et la thérapie par l'iode 131 sont recommandées pour les enfants et les adolescents ayant un carcinome de la thyroïde avec des métastases pulmonaires.

Resumen

Objetivo: Realizamos este análisis para evaluar los factores clínicos, el control tumoral local y las metástasis a distancia, la sobrevivencia y las complicaciones en niños y adolescentes con carcinoma del tiroides. Material y Métodos: Entre 1960 y 2003 tratamos por carcinoma del tiroides 21 niños y adolescentes (16 niñas y 5 niños de edades entre 4 y 15 años). Registramos los datos clínicos, los hallazgos operatorios y anatomopatológicos, los procedimientos operatorios y las complicaciones intra y postoperatorias así como el seguimiento que osciló entre 3 y 42 años (mediana 20 años). Resultados: Se encontró carcinoma papilar en 20 pacientes (95 %) y carcinoma folicular en 1. La incidencia de metástasis en los ganglios linfáticos cervicales fue de 12 sobre 21 (57,1 %) en el momento del diagnostico y 5 (23,8 %) tenían metástasis pulmonares. En los pacientes con metástasis pulmonares se hizo tiroidectomía total y resección radical de los ganglios linfáticos del cuello en ambos lados en 3 pacientes, lobectomía tiroidea y disección radical del cuello en un paciente y tiroidectomía parcial en otro. Tres niños en quienes se había realizado tiroidectomía total están vivos. Se usaron irradiación con yodo 131 y sustitución con hormona tiroidea en 2 de los 3 pacientes con metástasis pulmonares. Conclusiones: A pesar de las metástasis en los ganglios linfáticos e incluso en los pulmones, el pronóstico de los pacientes con carcinoma papilar del tiroides es bueno. Se recomiendan tiroidectomía total y terapia con yodo 131 en los pacientes con carcinoma tiroideo y metástasis.

Zusammenfassung

Zielsetzung: Ziel der vorliegenden Analyse war es, die Lokalisation, die Fernmetastasen, die Überlebensrate und die Komplikationen bei Kindern und Jugendlichen mit Schilddrüsenkarzinom zu überprüfen. Material und Methodik: Von 1960 bis 2003 wurden 21 Kinder und Jugendliche (16 Mädchen und 5 Knaben) im Alter von 4 - 15 Jahren wegen eines Schilddrüsenkarzinoms behandelt. Die klinischen Daten, die operativen Befunde, die histopathologischen Befunde, das Operationsverfahren, die intra- und postoperativen Komplikationen und der Verlauf wurden überprüft. Der Nachuntersuchungszeitraum betrug 3 - 42 Jahre (Durchschnitt 20 Jahre). Ergebnisse: Bei 20 Patienten (95 %) wurden papilläre Karzinome, bei einem Patient ein follikuläres Karzinom gefunden. Die Häufigkeit von zervikalen Lymphknotenmetastasen betrug 57,1 % (12 von 21 Patienten), 5 Patienten (23,8 %) wiesen Lungenmetastasen auf. Bei den Patienten mit Lungenmetastasen wurde eine vollständige Thyreoidektomie mit beidseitiger radikaler Lymphknotenresektion durchgeführt (3 Patienten), bei einem Patienten eine Lobektomie der Thyreoidea mit einer radikalen Lymphknotenresektion und bei einem weiteren Patienten eine partielle Thyreoidektomie. Drei der Kinder, die einer totalen Thyreoidektomie unterzogen wurden, waren bei den Verlaufskontrollen am Leben. Bei 2 der 3 Patienten mit Lungenmetastasen wurde eine Jod-131-Applikation bei gleichzeitigem Schilddrüsenhormonersatz durchgeführt. Schlussfolgerungen: Trotz Lymphknoten und sogar Lungenmetastasen ist die Prognose des papillären Schilddrüsenkarzinoms gut. Eine totale Thyreoidektomie und eine Jod-131-Therapie werden im Kindes- und Jugendalter beim Schilddrüsenkarzinom mit Lungenmetastasen empfohlen.
