Lung Morphology After Late Fetal Tracheal Ligation in Rats

Abstract

Pulmonary hypoplasia and persistent pulmonary hypertension are the main causes of mortality and morbidity in congenital diaphragmatic hernia (CDH). Prenatal tracheal occlusion accelerates lung growth, but the mechanism remains unknown. In order to be able to establish the accuracy of our experimental model for further molecular biological examinations, we evaluated the histologic structure of 1. fetal lungs subjected to tracheal occlusion compared to 2. normal fetal lungs, 3. hypoplastic lungs in CDH, and 4. normal neonatal lungs. One group of Sprague-Dawley rat fetuses were subjected to intrauterine tracheal ligation (TL) on gestational day 19 (n = 7). Control fetuses were obtained from the same litters as those subjected to TL (n = 8). Another group of pregnant Sprague-Dawley rats were given 100 mg nitrofen on gestational day 9.5 to create CDH (n = 8). All fetuses were delivered by cesarean section on day 21. Lungs from 1-day-old, healthy, non-operated, newborn Sprague-Dawley rats were also examined (n = 6). Lung weight to body weight ratio was significantly higher in the TL lungs (5.0 ± 0.36 %), compared to control lungs (2.8 ± 0.15 %), CDH lungs (1.9 ± 0.12 %), and normal neonatal lungs (4.2 ± 0.18 %). Volume density of alveolar air space and radial alveolar count (RAC) in TL lungs (52 ± 1.4 %) (3.3 ± 0.25) were significantly higher than in control lungs (34 ± 3.4 %) (2.2 ± 0.17) and in CDH lungs (16 ± 1.7 %) (1.7 ± 0.07). No significant differences were found between the TL and the normal neonatal group (59 ± 1.4 %) (3.6 ± 0.11). Fetal lungs after TL showed evidence of growth stimulation with increased volume density of alveolar air space and increased RAC, comparable to findings in normal neonatal lungs.

Buts: L'hypoplasie pulmonaire et l'hypertension pulmonaire persistante sont les causes principales de la mortalité et de la morbidité dans les hernies congénitales du diaphragme (CDH). L'occlusion trachéale prénatale accélère la croissance pulmonaire mais le mécanisme demeure inconnu. Dans le but d'établir la fiabilité de notre modèle expérimental pour des études en biologie moléculaire ultérieures, nous évaluons la structure histologique premièrement de poumons fœtaux soumis à une occlusion trachéale, deuxièmement de poumons fœtaux normaux, troisièmement de poumons hypoplasiques avec CDH, et quatrièmement des poumons néonataux normaux. Un groupe de fœtus de rats Sprague-Dawley a été soumis à une ligature intra-utérine de la trachée au 19ème jour de la gestation (n = 7). Les fœtus contrôle sont de la même race que ceux qui avaient été soumis à la ligature trachéale (n = 8). Un autre groupe de rats Sprague-Dawley enceintes recevaient 100 mg de Nitrofen au jour gestationnel 9,5 pour créer une hernie de coupole diaphragmatique (n = 8). Tous les fœtus étaient obtenus par césarienne au jour 21. Les poumons de rats nouveaux-nés non opérés Sprague-Dawley étaient aussi examinés (n = 6). Le rapport du poids pulmonaire au poids du corps était significativement plus élevé chez les poumons soumis à une ligature trachéale (5,0 ± 0,36 %), comparé au poumon contrôle (2,8 ± 0,15 %), aux poumons avec hernie de coupole diaphragmatique (1,9 ± 0,12 %), et aux poumons néonataux normaux (4,2 ± 0,18 %). Le volume de l'alvéole et le compte radial alvéolaire (RAC) chez les poumons soumis à une ligature trachéale (52 ± 1,4 %) était significativement plus élevé dans le groupe contrôle (34 ± 3,4 %) (2,2 ± 0,17) et dans les poumons avec hernies de coupole diaphragmatique (16 ± 1,7 %) (1,7 ± 0,07). Aucune différence significative n'a été trouvée entre les ligatures trachéales et le groupe néonatal normal (59 ± 1,4 %) (3,6 ± 0,11). Les poumons fœtaux après ligature trachéale montraient à l'évidence une stimulation de la croissance avec une augmentation du volume alvéolaire et du compte radial alvéolaire comparable à ceux trouvés chez un poumon néonatal normal.

Resumen

La hipoplasia pulmonar y la hipertensión pulmonar persistente son las causas principales de mortalidad y morbilidad en la hernia diafragmática congénita (CDH). La obstrucción traqueal prenatal acelera el crecimiento del pulmón por mecanismos aún desconocidos. Para establecer un modelo experimental para ulterior estudio biológico molecular evaluamos la estructura histológica de 1.- Pulmones fetales con tráquea ocluída en comparación con 2.- Pulmones normales, 3.- Pulmones hipoplásicos procedentes de CDH y 4.- Pulmones neonatales normales. Un grupo de fetos de rata Sprague-Dawley se sometió a ligadura traqueal intrauterina (TL) en el día 19 de gestación (n = 7). Los controles (n = 8) se obtuvieron de las mismas camadas. Otro grupo de ratas gestantes fue tratado con 100 mg. de Nitrofen en el día 9.5 para producir CDH (n = 8). Todos los fetos se obtuvieron por cesárea en el día 21. También fueron estudiados pulmones de ratas recién nacidas sanas (n = 6). El peso pulmonar en relación con el peso corporal fue significativamente más alto en los pulmones TL (5,0 ± 0,36 %) en comparación con los controles (2,8 ± 0,15 %), con los pulmones CDH (1,9 ± 0,12 %) y con los pulmones neonatales normales (4,2 ± 0,18 %). La densidad de volumen del espacio aéreo alveolar y el recuento alveolar radial en los pulmones TL (52 ± 1,4 %, 3,3 ± 0,25) fueron significativamente más altos que los de los controles (34 ± 3,4 %, 2,2 ± 0,17) y los CDH (16 ± 1,7 %, 1,7 ± 0,07). No hubo diferencias significativas entre los pulmones TL y los de grupo neonatal sano (59 ± 1,4 %, 3 ± 0,11). Era evidente que los pulmones fetales tras TL sufrieron estimulación del crecimiento con aumento de la densidad de volumen de los espacios alveolares y aumento del RAC siendo comparables a los pulmones neonatales normales.

Zusammenfassung

Zielsetzung: Lungenhypoplasie und persistierender pulmonaler Hochdruck sind die häufigsten Mortalitäts- und Morbiditätsursachen der angeborenen Zwerchfellhernien (CDH). Ein pränataler Trachealverschluss beschleunigt das Lungenwachstum, aber der pathophysiologische Mechanismus ist unklar. Um jedoch die Genauigkeit unseres experimentellen Modells für weitere molekularbiologische Untersuchungen zu überprüfen, untersuchten wir die feingewebliche Struktur von 1. fetalen Lungen nach Verschluss der Trachea im Vergleich zu 2. normalen fetalen Lungen, 3. hypoplastischen Lungen bei angeborener Zwerchfellhernie und 4. normalen Lungen von reifen Neugeborenen. Methode: Bei einer Gruppe von Sprague-Dawley-Ratten-Föten (n = 7) wurden am 19. Gestationstag intrauterine Trachealligaturen (TL) vorgenommen. Kontrolltiere (n = 8) wurden ohne Operation genauso versorgt und gepflegt wie die operierten Föten. Eine weitere Gruppe von Sprague-Dawley-Ratten (n = 8) erhielten am 9,5. Gestationstag 100 mg Nitrofen, um eine Zwerchfellhernie zu induzieren. Alle Tiere wurden am 21. Gestationstag per Kaiserschnitt entbunden. Darüber hinaus wurden die Lungen von gesunden, nicht operierten, 1 Tag alten neugeborenen Sprague-Dawley-Ratten untersucht (n = 6). Ergebnisse: Das Verhältnis von Lungengewicht zu Körpergewicht war bei den TL-Tieren (5,0 ± 0,36 %) signifikant höher als bei den Kontrolltieren (2,8 ± 0,15 %), den CDH-Lungen (1,9 ± 0,12 %) und den normalen Lungen der Neugeborenen (4,2 ± 0,18 %). Die Volumendichte des alveolären Luftraums und die radiale Zahl der Alveolen (RAC) war bei den TL-Lungen (52 ± 1,4 %; 3,3 ± 0,25) ebenfalls signifikant höher als bei den Lungen der Kontrolltiere (34 ± 3,4 %; 2,2 ± 0,17) und bei den CDH-Lungen (16 ± 1,7 %; 1,7 ± 0,07). Keine signifikanten Unterschiede fanden sich zwischen den TL-Tieren und den normalen neugeborenen Ratten (59 ± 1,4 %; 3,6 ± 0,11). Schlussfolgerungen: Fetale Lungen zeigen nach Trachealligatur eine Wachstumsstimulation mit Zunahme der Volumendichte des Alveolarraumes und Zunahme der RAC im Vergleich zu normalen Lungen von neugeborenen Ratten.
