Laparoscopic Nephroureterectomy in Children: A Prospective Study on Ligasure™ versus Clip/Ligation

Abstract

Purpose: The present study was performed to compare dissection of the renal hilar vessels in laparoscopic transabdominal nephrectomy in children using the Endo-Ligasure™ vessel sealing system versus clip/ligation. Patients and Methods: In a prospective and comparative study carried out from February 2003 to April 2004, 10 consecutive patients (group 1) underwent laparoscopic transabdominal nephroureterectomy using clips or intracorporeally performed ligations, respectively. From April 2004 to April 2005, 10 consecutive patients (group 2) underwent the same procedure using the Endo-Ligasure™ vessel sealing system. Indications for surgery were confirmed non-functioning kidneys secondary to benign unilateral renal disease and no prior surgery. The age and underlying disease distribution and the affected side were not significantly different between the two groups. Results: The operating time was significantly lower in the Endo-Ligasure™ group (group 1: median 167 vs. group 2: 108 min, p < 0.05). Bleeding of the renal artery occurred due to dislocation of a suture ligation, which was treated laparoscopically with an intracorporeal suture ligation. Blood loss was negligible in all patients. All procedures were completed laparoscopically and recovery was uneventful. Conclusions: Endo-Ligasure™ is a beneficial tool in laparoscopic transabdominal nephrectomy. It is safe, effective, and reduces operating times compared to clip application and intracorporeal suturing.

Buts: Cette étude a été réalisée pour comparer la dissection des vaisseaux du pédicule rénal au cours de néphrectomie laparoscopique transabdominale chez l'enfant en utilisant soit l'Endo-Ligasure soit des clips/ligatures. Patients et Méthodes: Dans une étude prospective et comparative de février 2003 à avril 2004, dix patients consécutifs (groupe 1) subirent une néphro-urétérectomie laparoscopique transabdominale en utilisant des clips ou des ligatures intra-corporelles. D'avril 2004 à avril 2005, dix patients consécutifs (groupe 2) subirent le même processus en utilisant l'Endo-Ligasure. Les indications pour la chirurgie étaient des reins non fonctionnels secondaires à une maladie rénale unilatérale bénigne et sans antécédent chirurgical. L'âge, la maladie et le côté affecté n'étaient pas significativement différents dans les deux groupes. Résultats: Le temps d'opération était significativement plus court avec l'Endo-Ligasure (groupe 1: médiane 167 versus groupe 2: 108 minutes, p < 0,05). Une hémorragie de l'artère rénale survint secondairement au lâchage d'une ligature qui avait été traitée laparoscopiquement avec une ligature intra-corporelle. La perte sanguine était négligeable chez tous les patients. Tous les procédés ont été réalisés par laparoscopie et les suites post-opératoires sans complications. Conclusion: L'Endo-Ligasure est un instrument très utile pour les néphrectomies laparoscopiques transabdominales. Il est sûr, efficace et réduit le temps opératoire comparativement aux applications de clips ou de sutures intra-corporelles.

Resumen

Objetivo: Este estudio pretende comparar la disección de las estructuras hiliares del riñón en la nefrectomía laparoscópica transabdominal en niños usando el sistema de sellamiento vascular endo-Ligasure™ contra la aplicación de clips/ligaduras. Pacientes y Métodos: En un estudio porspectivo comparativo realizado entre febrero del 2003 y abril del 2004 se trataron 10 pacientes consecutivos (grupo I) mediante nefroureterectomía laparoscópica transabdominal con clips o ligaduras intracorpóreas. Entre abril del 2004 y abril del 2005 fueron tratados 10 pacientes consecutivos (grupo II) por el mismo motivo usando el sistema de sellado vascular endo-Ligasure™. Las indicaciones quirúrgicas confirmaron riñones no funcionantes como consecuencia de enfermedades renales unilaterales sin cirugía previa. La edad y la enfermedad que originó la indicación y el lado afecto no fueron significativamente distintas en ambos grupos. Resultados: El tiempo operatorio fue significativamente menor en el grupo endo-Ligasure™ (167 vs 108 meses, p < 0,05). Ocurrió una hemorragia de la arteria renal debida a dislocación de la ligadura en el grupo tratado con ligadura intracorpórea pero que pudo ser tratada laparoscópicamente. La pérdida hemática fue mínima en ambos grupos. Todos los procedimientos se completaron laparoscópicamente y la recuperación fue sin incidentes. Conclusiones: El sellado con endo-Ligasure™ es un arma útil en la nefrectomía laparoscópica transabdominal. Es segura, eficaz y reduce el tiempo operatorio en comparación con la aplicación de clips y la sutura intracorpórea.

Zusammenfassung

Zweck der Studie: Die vorliegende Studie wurde durchgeführt, um die Dissektion des Nierenhilus bei der laparoskopischen Nephrektomie im Kindesalter mittels Endo-Ligasure™ mit der Verwendung von Clips/Ligaturen zu vergleichen. Patienten und Methodik: In einer prospektiven und vergleichenden Studie wurden initial 10 konsekutive Patienten (Gruppe 1) laparoskopisch von Februar 2003 bis April 2004 unter Verwendung von Clips bzw. Ligaturen nephrektomiert. Von April 2004 bis April 2005 wurden dann 10 konsekutive Patienten (Gruppe 2) unter Verwendung von Endo-Ligasure™ laparoskopisch nephrektomiert. Die Operationsindikation wurde bei einseitiger funktionsloser Niere auf dem Boden gutartiger Erkrankungen ohne vorherige Operation gestellt. Die Altersverteilung, der zugrunde liegende Erkrankungstyp und die Seitverteilung der betreffenden Nieren waren in unserem Krankengut nicht signifikant unterschiedlich. Ergebnisse: Die Operationszeit unter Verwendung von Endo-Ligasure™-Gruppe war signifikant kürzer (Gruppe 1: Median 167 min vs. Gruppe 2: 108 min, p < 0,05). Eine Blutung der Nierenarterie aufgrund einer dislozierten Ligatur konnte problemlos laparoskopisch mit einer erneuten Ligatur versorgt werden. Der Blutverlust war zu vernachlässigen. Alle Eingriffe wurden laparoskopisch durchgeführt bei jeweils unauffälligem postoperativem Verlauf. Schlussfolgerung: Endo-Ligasure™ ist ein hilfreiches Instrument bei der laparoskopischen Nephrektomie. Sein Einsatz ist sicher, effektiv und reduziert die Operationsdauer gegenüber der Verwendung von Clips und intraabdominell geknüpften Nähten.
