Successful Management of a Retroperitoneal Kaposiform Hemangioendothelioma with Kasabach-Merritt Phenomenon Using α-Interferon

Abstract

It has been shown recently that Kasabach-Merritt phenomenon, the association of a vascular tumour and consumption coagulopathy, does not - as previously thought - complicate “classical” infantile hemangiomas but distinctive entities called kaposiform hemangioendothelioma (KHE) and tufted angioma (TA), both tumours on the same neoplastic spectrum. These tumours have been found in the neck, face, thorax, abdomen, retroperitoneum and limbs and are associated with a mortality rate of as high as 30 %. Several therapeutic modalities, including α-interferon, vincristine, radiotherapy and surgery have been reported in the literature. We report a case of retroperitoneal kaposiform hemangioendothelioma regression using α-interferon and discuss the current knowledge of this entity and its treatment.

Résumé

Il a été montré récemment que le phénomène de Kasabach-Merritt, c'est à dire, l'association d'une tumeur vasculaire et d'une coagulopathie de consommation, ne complique pas, comme on le pensait auparavant, les angiomes infantiles «classiques» mais des entités distinctes appelées Hemangioendothéliome kaposiforme (HEK) et angiome en touffes (AT), tous étant aujourd'hui considérés comme faisant partie d'un même spectre tumoral. Celles-ci ont été décrites dans le cou, la face, le thorax, l'abdomen, le rétropéritoine et les membres avec une mortalité pouvant atteindre 30 %. De nombreuses modalités thérapeutiques dont l'interferon-α, la vincristine, la radiothérapie et la chirurgie ont été décrites. Nous décrivons un cas d'hémangioendothéliome kaposiforme rétropéritonéal traité par interferon et discutons de cette entité et de son traitement.

Resumen

Se ha demonstrado recientemente que el fenómeno de Kasabach-Merritt, asociación de tumor vascular y coagulopatia de consumo, no es una complicación como se creía de los hemangiomas infantiles clásicos sino de entidades claramente separadas denominadas hemangioendoteliomas kaposiformes (Khe) y angioma en penacho (Ta) y corresponden al mismo espectro neoplásico. Estos tumores han sido descritos en el cuello, la cara, el toráx, el abdomen, el retroperitoneo y los miembros y se han asociado con una mortalidad tan alta como 30 %. Se han propuesto varias modalidades terapéuticas incluyendo a interferon, vincristina, radioterapia y cirugía. Presentamos un caso de hemangiodetelioma kaposiforme retroperitonal que regresó usando interferon α y discutimos los conocimientos actuales sobre esta entidad y su tratamiento.

Zusammenfassung

Kürzlich konnte gezeigt werden, dass es sich bei dem Kasabach-Merritt-Syndrom, also der Assoziation von vaskulärem Tumor und Verbrauchskoagulopathie, nicht um eine Komplikation der klassischen kindlichen Hämangiome handelt, wie man bislang angenommen hatte, sondern um davon abzugrenzende Entitäten, die als kaposiformes Hämangioendotheliom (KHE) und büschelförmiges Hämangiom (TA „tufted angioma“) bezeichnet werden, die jedoch alle zur selben Gruppe von Neoplasien gerechnet werden. Diese Tumoren wurden im Bereich des Halses, des Gesichts, des Thorax, des Abdomens, des Retroperitoneums und der Extremitäten beschrieben und gehen mit einer Mortalitätsrate von bis zu 30 % einher. Verschiedene therapeutische Ansätze wie Interferon-α, Vincristin, Radiotherapie und chirurgische Behandlung sind in der Literatur beschrieben. Wir beschreiben in dieser Kasuistik ein retroperitoneales Hämangioendotheliom, welches mit Interferon-α behandelt wurde, und diskutieren den aktuellen Wissensstand bezüglich dieser Entität und seiner Behandlung.
